A resource guide designed to assist students with disabilities in South Mississippi in transitioning out of the school system to community engagement.

TRACK is a tool to assist the student and family in exploring options, making important decisions, and designing an individualized, step-by step, organized plan.TRACK equips, enables, and empowers students to achieve their maximum potential for self-determination, integration, and inclusion into their communities.
